ADMISSIONS

ADMISSION REQUIREMENTS: 

1. Original and photocopy of Birth Certificate from the Philippine Statistics       Authority (PSA) 

2. Original and photocopy of Baptismal Certificate as well as a waiver for           non-Catholics 

3. Photocopy of Report Card - most recent quarter grades (required for               transferees) 

4. Principal’s Certification of Good Ethical Character from past school                  (required for transferees for Grades 2 to 12 candidates only) 

5. Two ID pictures (1x1) 

6. Medical Certificate 

7. Medicate Test Result ( only for Grades 7-12 applicants)

ADMISSION PROCEDURE:

1. Yield necessities to the Registrar’s Office.

2. Fulfill the Application Form for Affirmation.

3. Pay the confirmation expense at the Cashier’s Office and return the                Application Form for Confirmation to the Registrar’s Office.

4. Go to the Guidance Office for the introductory interview, admission test        and final interview.

5. Secure Authorization from the Cashier to Enroll slip.
